Candidates must have the required qualifications to be considered for the position. The salary offer will be based on the candidate’s experience in the exact or comparable position and additional preferred qualifications will be considered.

Purpose of Position: Under the general supervision of the Supervisor, Housing Social Services, the Social Service Coordinator is responsible for providing case management and recreational services coordination for residents of a senior housing facility

Schedule:  Monday – Friday (Day Shift)

Salary: $47.5-50K annual DOE

Roles, Responsibilities and Essential Duties:

  • Actively coordinate day-to-day on-site services for residents and ensure all service provision is representative of RiseBoro’s mission
  • Conduct non-clinical annual assessments of residents to determine wellness and social needs
  • Provide ongoing case management, assistance, and advocacy, providing information and referrals to community services as needed
  • Assist clients with benefit entitlements i.e. SSA, SSI, SNAP, Public Assistance, Medicaid, Medicare, and private insurance
  • Educate residents on the available on-site and community services
  • Prepare and maintain resident records/personal information i.e., eligibility information, services provided, and all relevant correspondence
  • Conduct monitoring services through home visits and telephone calls
  • Provide on-site services weekly for a minimum of two (2) other assigned senior buildings, and coverage to additional sites as needed
  • Coordinate weekly planned activities and classes; observe classes for program evaluation, and change assessments
  • Ensure and encourage resident participation in educational workshops and provide proper translation of materials as needed
  • Communicate with on-site property management and maintenance staff on resident issues
  • Other related duties as required by the Supervisor

Required Skills/Qualifications:

  • High School Diploma or equivalent required
  • An associate or bachelor’s degree in social work, psychology, or a related field preferred
  • Basic computer skills with knowledge of database entry and email i.e. Outlook
  • Experience in customer or client services required
  • Bilingual Spanish required

  • Experience in social services setting preferred
  • Excellent verbal and written communication skills
  • Ability to multitask and problem-solve as needed
  • Ability to work collaboratively as a team and independently
  • Ability to develop positive relationships i.e. clients, family members, colleagues, etc.
  • Ability to maintain professionalism under stressful situations and maintain confidentiality
  • Ability to travel to different sites for home visits and appointments
