Salary Range:                  $45,000 - $55,000

Days/Hours:                     Day and Evening hours.

Position Summary:

This individual will be responsible for conducting initial orientation for new residents entering the Fortune Academy, develop relationships with DHS to identify and engage chronically homeless persons, facilitating groups for Fortune Society and carrying a manageable caseload for participants who are part of the Housing Department.

Essential Duties and Responsibilities:

• Conduct client groups in the areas of:
• Moral Recognition Therapy (MRT);
• Seeking Safety (Seeking Strength);
• Life Skills;
• Smoking Cessation;
• Additional Groups as Identified by Program Director, which may include Relapse Prevention, Anger Management, and Health Literacy;
• Carry a manageable caseload;
• Participate in case conferences as needed;
• Conducting toxicology;
• Outreach potential clients, whether via external partners or through interdepartmental collaboration
• Conduct and perform data entry for GPRA and SAMHSA Cross-Site surveys/evaluations at intake, 6-month follow-up, and discharge, maintaining appropriate follow-up rates;
• Use of the agency’s electronic database to record service-related information as follows;
• Printing out group attendance rosters before each group;
• Making sure that each client attending the group is scheduled, verifying this by looking at client’s copy of their schedule, Roster, or client agency ID;
• Entering individual IC and group session’s notes, conducting home visits, and service plans;
• Submitting signed group attendance rosters, on a daily basis, to the designated staff member;
• Inform their immediate supervisor of any barriers or challenges which may arise during the course of providing services;
• Participate in professional development and training via a self-directed means or as requested by supervisor; and
• Perform other related duties as needed.

Qualifications

Qualifications:

• High school degree or GED required with five years’ experience facilitating groups; BA or MSW preferred;
• Computer skills including Microsoft Office and familiarity with client management databases;
• Received training in, and have experience with, the fundamentals of group dynamics and methods of facilitation, and administering evidence-based treatment, preferably including Moral Recognition Therapy, and Seeking Safety;
• Experience facilitating culturally diverse groups that include young adults required;
• Be able to work independently and support a team approach;
• Strong oral and written communication skills required;
• Bi-lingual in English/Spanish a plus;
• Ability to work as part of a team;
• Knowledge of, and sensitivity to, social issues faced by formerly incarcerated men and women;
• Work schedule flexibility to provide group services during evening programming;
